Decision log

Purpose

Record the decision ID, responsible role and related task.

Records

Context and constraints; candidate approaches; choice and rationale; how assumptions were verified after generating an approach; rollback conditions; evidence paths.

Review

Review roles, comments, version digest and unresolved items; awaiting human completion.

Teaching feedback scaffolding

  • Observable behavior: (Describe an action the learner can demonstrate, its LO and verification evidence.)
  • Failed version of the same task: (Retain the input, generated result and evidence of failed verification.)
  • Diagnosis: (Use evidence to locate an assumption or implementation problem, rather than only copying an error message.)
  • Improved version: (Record revision differences; re-verify the same input and boundary inputs.)
  • Timebox and checkpoints: (Expected minutes; inspect artifacts at the checkpoint; when stuck, record the current state and ask for help.)
  • Hint ladder: (Begin with a question that locates the problem, then a related concept, then partial steps; record the level actually used.)
  • Formative feedback: (At least once; feedback source, time, evidence and actionable advice, to be completed.)
  • Visible revision: (At least once; before/after differences and re-verification results, to be completed; reuse the current file or existing activities.)
  • Teaching/rehearsal evidence: (Awaiting human completion; record pseudonymous roles, environment, timing, accessibility issues, failures and references to repair evidence.)

These fields serve teaching feedback only. They do not add weekly required files or change the eight-dimension rubric or grading contract. The template's existence, complete fields or passing automated checks do not mean that human teaching/rehearsal, calibration or approval has taken place.
